This form grants the Richmond
Police Department authority to check the criminal history of an individual.
Criminal background information is only accessible to the individual submitting
the request. The service fee for obtaining a criminal background report is $5.
When mailing a criminal history record request, please ensure the ''Affidavit
for Release of Information'' is signed and notarized. Please make your money
order or cashiers check payable to the City of Richmond, Police Records, 200 W. Grace Street,
Richmond, Va., 23220. Or you may bring your completed request to
the Richmond Police Department, Police Information area, 200 W. Grace Street
Valid identification will be required. Requests are processed Monday through
Friday, 8 a.m. until 5 p.m.
