Contact a local veterinarian and
arrange to have your pet receive a rabies vaccine. If your pet's rabies vaccine
is current, use the veterinarian's record to complete this form. You should
find the rabies number and the expiration date listed on these records or on
the rabies tag. Complete the form above and return it the Division of
Animal Control to obtain your City license. The cost of a City license is
$10.00.
